Enemies level up at the same rate as you do, so the game is consistently challenging.

On higher difficulties, spend more time dodging than attacking.

You can't drink in combat, so be sure to prep for battles through the Meditation menu.

They make combat easier, especially on the higher difficulty levels. Keep old final saves in sepearte folders with descriptive names, and possibly a readme.txt describing your choices in that playthrough. You can import it into The Witcher 3, and your choices will affect many of the side-quests, and some other subtle options.

Save just before choosing sides in Chapter I, so you don't have to repeat everything if you want to see how the other half lives.

Unlike the previous game, there is a quick save slot, so I do recommend making a 'hard' save every now and again.

It'd suck to screw one up only to have to reload from a slot you saved an hour or more ago.

I'll only mention this once: Save often.
